Referred Spinal Pain: The Differential Diagnoses

Oryon Develop are pleased to welcome Dr Gautham Srinivasan, Consultant in Pain Management, for this evening lecture. The first part of the talk will focus on differential diagnosis of referred spinal pain, and the second part on management and treatment options. Finally, the talk will focus on the importance of appropriate and timely onward referral of patients who fail to improve with conservative treatment.

Dr Srinivasan will firstly discuss the differential diagnosis between mechanical, neuropathic and complex pain. We will look at pain as presenting in the spine, neck, back, arms and legs. There will be a review of complex cases such as:

  • Failed back surgery syndromes
  • Radiculopathies
  • Complex regional syndromes
 The role of investigations will also be reviewed, including the following modalities and NICE guidelines:
  • MRI
  • blood tests
Dr Srinivasan will then move on to discuss the latest treatment approach including diagnostic injections, therapeutic injections, neuroaxial blocks and radiofrequency treatment.

We will also cover the role of rehabilitation and what to do when the patient fails to improve, including:
  • when to refer
  • when to resume rehabilitation
  • the importance of self-management


  • Spinal pain presenting in the neck, back, arms and legs
  • Differential diagnoses and what to look out for 
  • The role of investigations 
  • Up to date treatment options 


Gautham Srinivasan

Dr. Gautham Srinivasan is a consultant Anaesthetist with a special interest in pain medicine. He rotated as a special registrar in the Kings and St.Georges school of Anaesthesia and obtained a CCT in Anaesthesia and Advanced training in Pain Medicine.

Dr. Srinivasan currently practices at Croydon University Hospital for is NHS work.

He specializes in the management of neuraxial pain in the neck, shoulder, mid and lower back with sciatic type symptoms. He uses both fluroscopy and ultrasound to guide his interventions. He approaches the patient holistically and guides therapy most suitable for the individual.

Privately, he practices at North Downs Hospital.
