In this lecture we discuss how treating vascular disease in its various forms, such as Peripheral Arterial Disease (PAD) and Chronic Venous Insufficiency (CVI), can keep your patients walking.

1 in 4 people have some degree of venous reflux, associated with varicose veins or deep venous thrombosis. CVI affects both young and old and risk factors include immobilisation, long haul flights, pelvic or lower limb surgery, hormone contraceptives and pregnancy. 1 in 8 people over 50 have lower limb arterial disease, affecting mainly older males. We, therefore, examine connections of PAD with hypertension, hyperlipidaemia, diabetes, and smoking.


Vascular problems can cause swelling, discomfort, restless or tired legs and some people go on to develop skin changes, with loss of leg hair, eczema, lipodermatosclerosis, thrombopheblitis or ulceration or even pain while resting or gangrene. Miss Rudarakanchana aims to help us spot these signs and symptoms, and make a differential diagnosis using function tests and scans. For example, Buerger's test and ankle brachial pressure indices to assess the degree of blood flow impairment, and duplex ultrasound to show the exact pattern of reflux in deep and superficial veins and possible venous occlusion.


In the final half of the webinar, we delve into prevention and rehabilitation options. Although venous insufficiency is not preventable in many cases, symptoms may be improved by loss of weight and leg elevation. Arterial disease, on the other hand, may be prevented by lifestyle modifications including smoking cessation, BMI optimisation, low fat and salt diets, and cardiovascular exercise. 


Conservative management techniques can include the use of compression stockings, medication and supervised exercise classes in the early stages. However, others will benefit from superficial venous intervention in the form of endovenous laser ablation, foam sclerotherapy or avulsions, thrombolysis, and angioplasty with or without stenting. Lastly, we discuss the pros and cons to find a suitable treatment and the best possible outcome for your patients.

Nung Rudarakanchana

Consultant Vascular & Endovascular Surgeon | London

Miss Rudarakanchana qualified in 2004 from the Cambridge University MB PhD programme and was awarded the Lewin Prize in Surgery. She won numerous scholarships and fellowships during her PhD research in vascular cell biology, including the Magdalene College Donaldson Bye-Fellowship and the British Federation of Women Graduates Spurgeon Scholarship. She underwent higher surgical training in London, during which time she gained specialist training in open and endovascular repair of complex aortic disease at St Mary’s Hospital and the Royal Free Hospital and completed an Advanced Endovascular Fellowship in lower limb arterial disease and diabetic foot management at St Thomas’ Hospital.  


She was appointed as a NIHR Clinical Lecturer in 2011 at Imperial College, developing interests in endovascular skills training using immersive virtual reality simulation, optimization of endovascular surgery through procedure rehearsal, the use of cerebral embolic protection devices during endovascular aortic repair, and the metabonomics of aortic disease. Her current research projects include the use of nutrigenomics to reduce cardiovascular risk. Research programmes associated with these interests have generated half a million pounds in grants and won prizes in the UK, Europe and the USA. Miss Rudarakanchana has supervised BSc and PhD degrees. She is published widely and acts an invited reviewer for the British Journal of Surgery and the European Journal of Vascular & Endovascular Surgery. She is Vascular Lead for the NHS London Diabetic Foot Network and is currently completing a Masters in Healthcare Finance and Management.


Miss Rudarakanchana was appointed at London North West University Hospitals in 2019 and also practices at the London Clinic and Wellington Hospital. Her clinical interests include aortic aneurysm and dissection, peripheral arterial disease and diabetic foot management (including complex wound management), carotid artery disease and TIAs/stroke, venous disease (including varicose veins, pelvic congestion and post-thrombotic limb), lymphoedema, thoracic outlet syndrome and women’s cardiovascular risk management. She undertakes both open and endovascular procedures, including complex endovascular aortic repair and lower limb angioplasty.
