Interview with Doctor Naeem

We sat down with our very own Dr Naeem to get the answers to some of our patients’ frequently asked questions. When should someone get an MRI for joint pain instead of an X-Ray?

DR:  Your referrer might request an MRI for a number of reasons. One of those might be to better understand the extent of the injury, or it might be useful in planning your surgery.

How do you tell the difference between a sprain, strain, or tear?

DR:  Radiologists can assess sprains, strains and tears using a number of different classification systems. We would be experienced in looking at these musculoskeletal MRIs and using the classification system. We would be able to derive a grading.

Can Imaging tell how long an athlete will be out of play?

DR: Usually, imaging can help determine the extent of a muscle injury or a sports injury. We use that in conjunction with the clinical assessment to help make some recommendations about return to play.

How can you tell the difference between new and old injuries?

DR: Radiologists are experienced in reporting and looking at scans, and part of the way they can differentiate between older and new injuries is by looking for signs of oedema or fluid, which would really indicate that.

This is an injury that has happened to the bone or muscle, or soft tissue, more recently than old.

Can you tell us about how Ultrasound guided injections work?

DR: At Oryon, predominantly, the ultrasound-guided injections are two things. One is an ultrasound-guided injection using steroids, and the other is an ultrasound-guided injection using something called hydronic acid. Now it would usually be a conversation with the referrer. And looking at the clinical picture in deciding which injection is most suitable for the patient.

When would you use an Ultrasound scan over an MRI?

DR: Ultrasound can be useful for dynamic assessments. So, for example, if we are assessing the shoulder, we’re able to assess for something like impingement dynamically, which can be useful in getting a diagnosis versus a predominantly static exam, such as an MRI.

We’ll also be offering prostate screening from September 2025 onwards, so we asked Dr Naeem some commonly asked questions about this type of imaging service. 

How can an MRI help check for prostate problems?

DR: An MRI provides us with details on your prostate. It allows us to not only look at the prostate itself, but also all the surrounding structures. And with that detail, we can then determine if there are any suspicious lesions and give them a grading. And the grading, in conjunction with your blood tests and a clinical discussion, will then determine what next steps are taken.

Is a Prostate MRI uncomfortable or invasive in any way?

DR: The prostate MRI scan is completely non-invasive.

It will require a period of time within the scanner, and during that time, you’ll be in contact with the radiographer, who will ensure that you are comfortable and will be talking to you throughout the scan.

You are also usually given a set of earphones which are playing any selected music or radio station that you’d like, and the idea is to try and keep you as still and as comfortable as possible so we can get the best possible images.
